Collecting relevant information about a patient’s medical history and current condition is essential to providing patients with the appropriate treatment. Be sure to bring all relevant medical information to your consultation, along with a list of prescriptions you are currently taking and allergies you have to any medication. Along with a list of prescriptions, including any over-the-counter, herbal supplements, or vitamins you take regularly. Our team may also ask you questions about your level of activity and health and lifestyle habits. This patient information will allow us to develop a personalized treatment plan for your unique needs and preferences. You can fill out this information in our “New Patient Information Form” before the appointment.
